The Florida Department of Health in Broward County leads the community health assessment and planning process in Broward County along with community partners.  DOH-Broward engaged the community in the Mobilizing for Action through Planning and Partnerships Process (MAPP). A broad range of community partners participated including: business, child care agencies, community based organizations, educational institutions, faith based institutions, funders, governmental agencies, healthcare providers, law enforcement, mental health and volunteer clinics.  MAPP is a strategic approach to community health improvement and assists communities to improve health and quality of life through community-wide strategic planning.  MAPP includes four separate assessments: Community Health Status Assessment, Community Themes and Strengths Assessment, Local Public Health System Assessment and the Forces of Change Assessment.

Through the MAPP process, the community identified 19 challenges which were prioritized, and eight priorities were selected, and organized into four priority areas.

  1. Increase Access to Health Services
    1. Barriers to health care access including lack of health insurance
    2. Public health planning  
  2. Reduce the Incidence of Communicable and Infectious Disease
    1. HIV/AIDS
    2. Sexually Transmitted Diseases (including Infectious and Congenital Syphilis)
  3. Improve Maternal, Infant and Child Health
    1. Black Infant Mortality
    2. Immunization for Kindergarten and 7th grade students
  4. Enhance Preventive Care Activities
    1. Cervical cancer screening
    2. Obesity/Food Security/Hunger/Nutrition

Based on these priorities a Community Health Improvement Plan (CHIP) was developed.  The plan is reviewed and updated on a quarterly basis, and quarterly and annual progress reports are produced.
